When you call to schedule an appointment please have your prescription in front of you as you will be asked for the following information:
- Type of exam ordered
- Diagnosis
- Information on any prior films of the area being scanned
- Your referring physician's name, address, phone and fax number
- Patient name, address, home or business contact number
For your convenience we can schedule exams early mornings, evenings and weekends for many procedures. To schedule an appointment by phone, please call (212-772-3111) between the hours of 8am to 7pm, Monday through Friday.
