Procedure for National Oncologic PET Registry (medicare non-covered diagnosis, indications below)
- Please schedule appointment with diagnosis at Lenox Hill Radiology.
- Lenox Hill Radiology will register cases with NOPR and will receive the case number.
- Lenox Hill Radiology will send Pre-PET form to PET registry and send a copy to the referring physician.
- PET scan must be scheduled with the patient within 14 days of case registration (most cases will be scheduled within the next day)
- PET scan report will be sent to referring doctor.
- A copy of the POST-PET form will be sent to the referring physician. This form must be completed by the referring physician in order to get re-imbursed for exams. Please fax POST-pet form to our billing department (212) 861-1796.
