At Lenox Hill Radiology, we realize your time is valuable. To minimize your waiting time, you may complete the registration forms in advance. You can print the appropriate form(s) and bring them completed with you to your appointment or fax them to us.
If you prefer, you can of course complete the form when you arrive for your appointment. If you have any questions about any form please call our office at

All patients are asked to fill out these forms prior to their exam, even patients who have had previous exams at Lenox Hill. Although we keep all your prior information and refer to it, we note any changes in information, as this is critical for the interpretation of your exam.
Please note: You are not required to fill out these forms for the following exams - Digital X-ray, Digital Fluoroscopy, Sonography/Ultrasound.
